Targeted case management helps eligible individuals with significant behavioral health needs organize care, access services and address barriers that may interfere with stability and recovery.
Support May Include
• Comprehensive needs assessment
• Individualized service planning
• Coordination among providers and support systems
• Referrals to medical, behavioral health and community resources
• Advocacy related to treatment and essential services
• Monitoring progress and following up on referrals

Targeted case management is subject to eligibility, medical necessity and payer or program requirements. It is not emergency response, transportation, housing placement or direct financial assistance.
