Introduction:

At Agape Health and Wellness Center we provide internal medicine care with select

Infectious Disease (ID) services, primary care (PC) services, and prevention services for

individuals in Jacksonville and across the state of FL. We provide respectful, non-

judgmental care to people of all backgrounds.

Which select ID and PC services do we offer?:

ID: Our ID services are specific to detecting, treating, and preventing HIV, other sexually

transmitted infections (STIs: gonorrhea, chlamydia, trichomonas, syphilis, herpes,

mycoplasma genitalium), and HepC. We provide comprehensive ongoing management

with antiretroviral, antiviral, and antibiotic therapy, routine monitoring, education, and

follow-up. For those who are HIV-positive, we provide services whether you are newly

diagnosed, returning to care, or transferring care. We also provide preventive care services,

including PrEP (pre-exposure prophylaxis for HIV) and Doxy-PEP for patients’ partners and

for those who are HIV negative.

PC: These services evaluate, treat, and manage common illnesses, health concerns, and

chronic conditions. It typically includes:

• Wellness visits and physical exams

• Screening and preventive care

• Lab/blood work

• Prescriptions

• Referrals and coordination with other specialists, case management, and

behavioral health

Why Having Both in One Place Matters:

When ID and PC services are under one umbrella, your health is comprehensive. You do

not have to wonder who is keeping track of your care.

Prevention:

AHAWC provides screening, education, and prevention support to protect you, your family, and your community. We provide PrEP and Doxy-PEP for those who are HIV negative and/or have HIV-positive or multiple sexual partners.

We maintain a clean clinic environment with use of appropriate disinfection and PPE

(personal protective equipment).

How We Can Help:

People come to us with many different concerns. Here are some of the most common:

"I need a doctor who takes my insurance."

We accept most insurances. Contact us and we will verify your insurance and benefits.

"I want to get tested privately."

Confidentiality is central to how we provide care. Your health information is protected, and our team will explain your privacy rights before you begin.

"I think I may have been exposed."

If you believe you have been exposed to an infection, do not wait. Contact a health care

provider as soon as possible. For certain exposures, including HIV, time matters, and our

provider can tell you what to do next.

"I have a new diagnosis and I'm not sure what to do."

You do not have to figure it out alone. We can help you understand your diagnosis, discuss

treatment options, and stay connected to ongoing care and support.

Your Privacy Matters:

We understand that many people worry about privacy when seeking testing or care for an infection. We follow federal and state privacy laws, including HIPAA, and we handle your information with care. During your visit, we will explain how your information is protected and the limited situations, required by law, in which certain information may need to be reported or shared.

Is testing confidential?

Yes. We follow federal and state privacy laws, including HIPAA,

and will explain how your information is protected.

Will my employer or family find out? Your information is not shared without your

permission, except in limited situations required by law, which we will explain to you.

Will I be judged? No. Our team provides respectful, non-judgmental care to everyone.
